Operational Failure

Lack of documentation and communication prevented teammates from intervening during the production failure. A technically sound migration can still fail when operational planning and communication are weak. Overly clever implementation can become unmaintainable for the surrounding team. Production incidents should be tracked and converted into preventative measures.
